KidsWork Children's Museum asks that you provide one (1) adult chaperone per ten (10) school age children and one (1) adult chaperone per six (6) pre-school age children.

By submitting this form, you agree that you will be responsible for the conduct of the group, which has been granted the use of the museum and shall conform to the official operation policies relating the building and property as established by the museum.

A security deposit of $50 is required upon booking your field trip. Security deposits are non-refundable and will be applied to total costs at the completion of your field trip. The total amount will be due on the day of the Field Trip.
